ABSTRACT

Objective: To investigate the factors influencing total bilirubin elevation and its correlation with UGT1A1 gene polymorphism in the early postoperative period of transjugular intrahepatic portosystemic shunt (TIPS). Methods: 104 cases with portal hypertension and esophageal variceal hemorrhage (EVB) treated with elective TIPS treatment were selected as the study subjects and were divided into a bilirubin-elevated group and a normal bilirubin group according to the total bilirubin elevation level during the early postoperative period. Univariate analysis and logistic regression were used to analyze the factors influencing total bilirubin elevation in the early postoperative period. PCR amplification and first-generation sequencing technology were used to detect the polymorphic loci of the UGT1A1 gene promoter TATA box, enhancer c.-3279 T > G, c.211G > A, and c.686C > A. Logistic regression was used to analyze the correlation of four locus alleles and genotypes with elevated total bilirubin in the early postoperative period. Results: Among the 104 cases, 47 patients were in the bilirubin elevated group, including 35 males (74.5%) and 12 females (25.5%), aged (50.72 ± 12.56) years. There were 57 cases in the normal bilirubin group, including 42 males (73.7%) and 15 females (26.3%), aged (51.63 ± 11.10) years. There was no statistically significant difference in age (t = -0.391, P = 0.697) and gender (χ(2) = 0.008, P = 0.928) between the two groups of patients. Univariate analysis revealed that preoperative alanine transaminase (ALT) level (χ(2) = 5.954, P = 0.015), total bilirubin level (χ(2) = 16.638, P < 0.001), MELD score (χ(2) = 10.054, P = 0.018), Child-Pugh score (χ(2) = 6.844, P = 0.022), and postoperative portal vein branch development (χ(2) = 6.738, P = 0.034) were statistically significantly different between the two groups. Logistic regression analysis showed that preoperative ALT level, total bilirubin level, and portal vein branch development after TIPS were correlated with the elevated total bilirubin in the early postoperative period. The polymorphism of the c.211G > A locus of the UGT1A1 gene correlation had elevated total bilirubin in the early postoperative period of TIPS. The risk of elevated total bilirubin was increased in the population carrying allele A (P = 0.001, OR = 4.049) in the early postoperative period. Allelic polymorphisms in the TATA box promoter region and enhancer c.-3279 T > G and c.686C > A had no statistically significant difference between the bilirubin-elevated group and the normal bilirubin group. Conclusion: The preoperative ALT level, total bilirubin level, and portal vein branch development are correlated with the elevated total bilirubin in early postoperative patients. The polymorphisms of the UGT1A1 gene and enhancer c.211G > A are correlated with the occurrence of elevated total bilirubin in the early postoperative period of TIPS. Allele A carrier may have a higher risk of elevated total bilirubin in the early postoperative period.

ABSTRACT

Drug-induced bile duct injury is a specific kind of drug-induced liver injury that has two main pathological types, namely ductopenia, or vanishing bile duct syndrome, and secondary sclerosing cholangitis. However, in recent years, the reports of new drugs that cause bile duct injury have been constantly increasing, and these drugs have different clinicopathological features and a novel pathogenesis. Therefore, this paper summarizes and analyzes the progress and challenges in the etiology, pathogenesis, diagnosis and treatment, and other aspects of drug-induced bile duct injury.

ABSTRACT

Objective: To investigate the incidence and treatment of perioperative anemia in patients with gastrointestinal neoplasms in Hubei Province. Methods: The clinicopathological data of 7 474 patients with gastrointestinal neoplasms in 62 hospitals in 15 cities (state) of Hubei Province in 2019 were collected in the form of network database. There were 4 749 males and 2 725 females. The median age of the patients was 62 years (range: 17 to 96 years). The hemoglobin value of the first time in hospital and the first day after operation was used as the criterion of preoperative anemia and postoperative anemia. Anemia was defined as male hemoglobin <120 g/L and female hemoglobin <110.0 g/L, mild anemia as 90 to normal, moderate anemia as 60 to <90 g/L, severe anemia as <60 g/L. The t test and χ2 test were used for inter-group comparison. Results: The overall incidence of preoperative anemia was 38.60%(2 885/7 474), and the incidences of mild anemia, moderate anemia and severe anemia were 25.09%(1 875/7 474), 11.37%(850/7 474) and 2.14%(160/7 474), respectively. The overall incidence of postoperative anemia was 61.40%(4 589/7 474). The incidence of mild anemia, moderate anemia and severe anemia were 48.73%(3 642/7 474), 12.20%(912/7 474) and 0.47%(35/7 474), respectively. The proportion of preoperative anemia patients receiving treatment was 26.86% (775/2 885), and the proportion of postoperative anemia patients receiving treatment was 14.93% (685/4 589). The proportions of preoperative anemia patients in grade ⅢA, grade ⅢB, and grade ⅡA hospitals receiving treatment were 26.12% (649/2 485), 32.32% (85/263), and 29.93% (41/137), and the proportions of postoperative anemia patients receiving treatment were 14.61% (592/4 052), 22.05% (73/331), and 9.71% (20/206). The proportion of intraoperative blood transfusion (16.74% (483/2 885) vs. 3.05% (140/4 589), χ²=434.555, P<0.01) and the incidence of postoperative complications (17.78% (513/2 885) vs. 14.08% (646/4 589), χ²=18.553, P<0.01) in the preoperative anemia group were higher than those in the non-anemia group, and the postoperative hospital stay in the preoperative anemia group was longer than that in the non-anemia group ((14.1±7.3) days vs. (13.3±6.2) days, t=5.202, P<0.01). Conclusions: The incidence of perioperative anemia in patients with gastrointestinal neoplasms is high. Preoperative anemia can increase the demand for intraoperative blood transfusion and affect the short-term prognosis of patients. At present, the concept of standardized treatment of perioperative anemia among gastrointestinal surgeons in Hubei Province needs to be improved.

ABSTRACT

Objective • To explore the preliminary experience of the construction of national major science and technology infrastructure for translational medicine in China, and to provide reference for their future development. Methods • The current situation, experience and enlightenment of major science and technology infrastructure construction at home and abroad were sorted out by literature analysis, relevant recommendations were collected through expert interviews, and the preliminary experience of national major science and technology infrastructure construction for translational medicine in China was summarized. Results • The experience and enlightenment that can be used for reference from the construction of major science and technology infrastructure at home and abroad mainly included 7 aspects, i.e. high-quality team construction, device sharing, industry-academiaresearch collaboration, international cooperation, the cluster effect producing, talents cultivation and the scientific data sharing mechanism. In China, the construction of major science and technology infrastructure for translational medicine started late, and had a good try at the above first 5 aspects, but not so good at the last 2. At the same time, self-experience has been formed in taking full advantages of the supporting institutions and getting funds through multiple channels. Conclusion • The construction of national major science and technology infrastructure for translational medicine in China should learn from the development experience at home and abroad, combine our own characteristics, explore and innovate the system and mechanism, and seek new breakthroughs.

ABSTRACT

Objective • To compare the current situation and propose the development plan for the clinical specialties of Shanghai Jiao Tong University School of Medicine. Methods • Based on Scopus database, English academic papers issued by 39 clinical specialties of Shanghai Jiao Tong University School of Medicine during 2011-2015 were analyzed by bibliometric methods. Results • The overall performance of the clinical specialties was good. The total scholarly output was 8 957 and showed an increasing trend year by year, the total citation count was 99 342, the total average citation count was 11.1, the total number of Top 1% of highly cited papers was 83, the total international cooperation papers accounted for 16.3%, and the total field-weighted citation impact of 22 clinical specialties was higher than the world average level. However, the development of each clinical specialty was not balanced. According to the number of the outstanding indexes, the 39 clinical specialties could be divided into five types, including leading type, advantage type, potential type, characteristic type, and disadvantaged type. Conclusion • According to the different types of clinical specialties, medical colleges should adhere to the development concept of classified management and take targeted measures to promote the overall level of clinical specialties.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate clinical effects of calcaneal fracture with closed reduction and minimally invasive plate fixation assisted with bidirectional distractor distraction.</p><p><b>METHODS</b>From September 2015 to October 2016, 11 male patients(13 feet) with calcaneal fractures treated with bidirectional distractor distraction assisted with minimally invasive plate fixation were retrospectively studied. They were aged from 24 to 57 years old with an average of 36.4 years old;8 feet were type IIand 5 feet were type III according to Sanders classification. Postoperative incision, fracture healing, Böhler angle, Gissane angle were observed and Maryland scoring system was used to evaluate clinical effects.</p><p><b>RESULTS</b>All fractures healed well without incision inflammation and incision disunion. All patients were followed up from 12 to 15 months with an average of 13.5 months. Böhler angle were improved from (9.6±7.3)° before operation to (20.2±4.6) ° at 1 year after operation, and had statistical meaning; Gissane angle increased from (92.7 ±8.5)° before operation to (121.7 ±7.6) ° at 1 year after operation. Maryland score at 1 year after operation was 88.79±8.25, and 11 feet got excellent results and 2 feet moderate.</p><p><b>CONCLUSIONS</b>Bidirectional distractor distraction assisted with minimally invasive plate fixation could effectively fix calcaneal fractures, reduce postoperative complications, and get satisfied results of postoperative images and functional recovery. It is one of effective methods for treating Sanders II and III calcaneal fractures.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyse the imaging characteristics of the irreducible femoral intertrochanteric fracture and evaluate the operative effect of closed poking reduction and intramedullary nail fixation.</p><p><b>METHODS</b>From December 2011 to October 2015, 28 patients with irreducible femoral intertrochanteric fracture were treated by closed poking reduction and intramedullary nail fixation including 12 males and 16 females with an average age of 72 years old ranging from 58 to 89 years old. According to AO classification: 5 cases were type A2.1, 10 cases were type A2.2, 13 cases were type A3.3. The postoperative complications were observed, and the postoperative X ray were used to evaluate the fracture healing, and the Harris score was used to evaluate the hip joint function.</p><p><b>RESULTS</b>Preoperative X rays for determine irreducible femoral intertrochanteric fracture was difficult, but dimensional CT had characteristic performance: the fracture near the end displaced forward rotation. Operation time was 45 to 100 minutes with an average of 65 minutes. Intraoperative bleeding was 80 to 300 ml with an average of 160 ml. Two cases died within 3 months after operation, 26 patients were followed up for 6 to 12 months with an average of 8.4 months. Fracture healing time was 12 to 16 weeks with an average of 13.6 weeks. Two cases occurred inversion deformity.There were no wound infection, nonunion and fracture fixation failure. Hip Harris score was 86.7±5.3 at the last follow up, the result was excellent in 14 cases, good in 12 cases.</p><p><b>CONCLUSIONS</b>Dimensional CT can be very good to predict the fracture of the irreducible femoral intertrochanteric fracture. Closed poking reduction and intramedullary nail fixation for the treatment of the irreducible femoral intertrochanteric fracture has satisfactory clinical result.</p>

ABSTRACT

Objective To investigate the expression of T-cadherin in hepatocellular carcinoma and its relationship with tumor relapse and metastasis. Methods The gene expressions of T-cadherin in hepatocellular carcinoma tissue with different malignancy grades and its matched adjacent tissues were assessed by immunohistochemistry. The samples were obtained from patients both with relapse and/or metastasis, without relapse, and without metastasis within one year post-operation, and they were in the same TNM stage before operation. The mRNA expression of T-cadherin in hepatocellular carcinoma and matched adjacent tissues was assessed by RT-PCR. Results The abnormal expression rate of T-cadherin in hepatocellular carcinoma was 46.67% (28/60), which was significantly higher than that of the adjacent tissue (6.67%, 4/60, χ2=24.55, P=0.000). No difference was observed in the expression of T-cadherin between well-differentiated and poorly differentiated hepatocellular carcinoma tissues (30% vs 35%, χ2=0.114, P=0.736). The abnormal expression rate of T-cadherin in hepatocellular carcinoma in the patients with metastasis elevated to 75% (15/20), while it was remarkably lower in 32.5% (13/40) of the patients who had no metastasis (χ2=9.676, P=0.002). The RT-PCR results showed that the mRNA expression of T-cadherin in hepatocellular carcinoma significantly decreased compared with that of its adjacent tissues. Conclusions There is an abnormal expression of T-cadherin in hepatocellular carcinoma, and it is found to be closely correlated to the tumor recurrence and metastasis. Abnormal expression of T-cadherin in hepatocellular carcinoma may be used as a prognostic indicator and drug research target.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the molecular mechanism of erythrocyte pyruvate kinase deficiency (PKD).</p><p><b>METHODS</b>Targeted sequence capture and next-generation sequencing (NGS) were used to detect the regions of exon and exon-intron boundarie of PKLR gene in a clinical suspected PKD patient. The protein function of mutant gene was forecasted by the SIFT and PolyPhen-2 databank, after the mutation of PKLR gene in the patient was detected by the NGS technology, its genotype was confirmed by Sanger sequencing.</p><p><b>RESULTS</b>The patient was found to have peculiar double heterozygous mutations: 661 G>A (Asp221Asn) of exon 5 and 1528 C>T (Arg510Ter) of exon 10, resulting in amino acid substitution Asp221Asn and Arg510Ter, these mutations were also further confirmed by Sanger sequencing. The complex mutations were infrequent and each of them was able to cause diseases.</p><p><b>CONCLUSION</b>The complex mutations of both 661 G>A and 1528 C>T of PKLR gene are the molecular mechanism of PKD. Simultaneous existance of above-mentioned complex mutations in PDK patient was never been previously reported at home and abroad.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the neurological protective effects of progesterone (PROG) on focal cerebral ischemia/reperfusion injury in rats and to explore its possible mechanism.</p><p><b>METHODS</b>One handred and twenty male SD rats were divided into three groups randomly: sham-operated group, middle cerebral artery occlusion ( MCAO ) group and PROG + MCAO group( n = 40). The right temporary MCAO model was established by the line-embolism method. The PROG + MCAO group rats were according to 8 mg/kg intraperitoneal injection PROG, after that 30 min, the rats were suffered ischemia/reperfusion. After rats were suffered ischemia for 2 h and reperfusion 0, 24, 48, 72 h stress, the nervous functional defect degree were evaluated by longe scoring, and the expression of two-pore domain K channel 3 (TASK3) mRNA in brain tissue were detected by the real-time PCR.</p><p><b>RESULTS</b>PROG (8 mg/kg) could significantly reduced the nervous functional defect degree in rats after ischemia/reperfusion 24, 48, 72 h (P < 0.05). The results of real-time PCR showed that the TASK3 mRNA expression in the brain tissue at all time points significantly decreased in MCAO group compared with sham-operated group (P < 0.05). However, compared with MCAO group, the expression of TASK3 mRNA in brain tissue at all time points dramatically increased in PROG + MCAO group (P < 0.05).</p><p><b>CONCLUSION</b>PROG can improve the nervous functional defect degree after focal cerebral ischemia/reperfusion injury in rats, and the mechanism might be associated with up-regulating the expression of TASK3 mRNA in brain tissue.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To construct the acid-sensitive potassium hannel-3(TASK3) eukaryotic expression plasmid and to establish a stable SH-SY5Y cell line expressing enhanced green fluorescent protein (EGFP)-tagged TASK3.</p><p><b>METHODS</b>TASK3 coding region was subcloned into pEGFP-N1 plasmid to construct a recombinant vector alled pEGFP-TASK3. The correct recombinant expressing plasmid was transfected with X-feet transfection reagent to SH-SY5Y cells. The cell line stably expressiing EGFP tagged-TASK3 gene was established by screening with antibiotic G418 and fluorescence microscope. The expression and localization of the EGFP tagged-TASK3 fusion protein was detected by Western blot and confocal microscope. Exposure of the SH-SY5Y cell line expressing stably TASK3-eGFP fusion proteins was exposed to different pH media (7.0, 6.7, 6.4, 6.1) for 24 h, the cell viability was assessed with cell counting Kit-8 (CCK-8).</p><p><b>RESULTS</b>All the results of identification by PCR, digestion with restriction endonuclease and sequencing indicated that the recombinant eukaryotic expression plasmid pEGFP-TASK3 was constructed correctly. The stable SH-SY5Y cell line expressing EGFP tagged-TASK3 fusion protein was successfully established. Exposure of the wild type SH-SY5Y cells and the stable SH-SY5Y-GFP tag-TASK3 cell line to different pH media (7.0, 6.7, 6.4, 6.1) for 24 h, the cell viability of two group cells significantly reduced with pH declining, and the difference was statistically significant (P < 0.05). Compared with wild type SH-SY5Y cells, the cell viability of stable SH-SYSY-GFP tag-TASK3 cell line increased significantly with the same pH media, and the difference was statistically significant (P < 0.05).</p><p><b>CONCLUSION</b>The eukaryotic expression vector pEGFP-TASK3 is successfully constructed and the cell line stably expressing TASK3-eGFP fusion is established which is important for their fundamental research and potential applications.</p>

ABSTRACT

<p><b>BACKGROUND</b>Following the increased human immunodeficiency virus (HIV) notification (infection) rates via sexual transmission and increased abuse of new synthetic drugs instead of heroin in China, a study on HIV infections and risk factors among synthetic drugs and heroin users in Beijing was conducted to provide a cue for further development of targets and strategies of HIV/AIDS control and prevention for different drug populations.</p><p><b>METHODS</b>A total of 428 subjects including 224 heroin users and 204 new synthetic drug users was recruited from communities in Beijing by convenience sampling. A χ(2) test was used to compare HIV and syphilis infections between the two subpopulations. Logistic regression models were used to evaluate factors related to HIV, syphilis infections and abuse of synthetic drugs.</p><p><b>RESULTS</b>Unbalanced distributions of demographics were detected between the two subpopulations. Compared with heroin users, most users of synthetic drugs were migrants, single and of younger age (age ≤ 30 years). Ten HIV infections and twenty-four syphilis infections were confirmed among 428 subjects. No significant difference in HIV infection was found between the two subpopulations (crude odds ratio (OR) = 0.46, 95% confidence interval (CI): 0.07 - 15.80 and adjusted odds ratio (AOR) = 1.26, 95%CI: 0.09 - 18.28) while four times as many syphilis infections were identified among users of synthetics drugs (AOR = 3.92, 95%CI: 1.06 - 14.56). HIV and syphilis infections appeared to work synergistically. The migrants who were of Han ethnicity, single and who had stayed in Beijing for more than six months were more likely to abuse synthetics drugs.</p><p><b>CONCLUSIONS</b>The new synthetic drug users and heroin users are different subpopulations. We optimistically speculate that HIV has not yet been introduced into the former in Beijing, but considering higher syphilis infections in synthetic drug users and the role of syphilis in HIV infection, synthetic drug users, as a possible and important driver of the HIV/AIDS epidemic in Beijing, should be paid more attention in HIV prevention and control strategies.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of progesterone (PROG) on learning and memory and P2X7 receptor expression in the hippocampus after global cerebral ischemia/reperfusion (I/R) injury in rats.</p><p><b>METHODS</b>Forty-eight male SD rats were randomly divided into four groups (n = 12) that were normal group, sham-operated group, I/R group and I/R+ PROG group. The global cerebral I/R injury models were established by improved Pulsinelli's four vessel occlusion, the learning and memory were evaluated by Y-maze, the expression of P2X7 receptor protein in the hippocampus were detected by the immunofluorescence, the activity of superoxide dismutase (SOD) in the hippocampus were detected with hydroxylamine oxidation method, the content of malondialdehyde (MDA) in the hippocampus were detected with pen-thiobarbituric acid method.</p><p><b>RESULTS</b>There was no significant difference of the learning and memory and positive expression cells of P2X7 receptor protein and the activity of SOD and the content of MDA in the hippocampus between normal group and sham-operated group. Compared with sham-operated group, the learning and memory of I/R group were significantly decreased (P < 0.01), Compared with I/R group, the learning and memory of I/R + PROG group were significantly increased (P < 0.05). Compared with sham-operated group, the positive expression cells of P2X7 receptor protein in the hippocampus of I/R group was significantly increased (P < 0.01), Compared with I/R group, the positive expression cells of P2X7 receptor protein in the hippocampus of I/R + PROG group was significantly decreased (P < 0.01). Compared with sham-operated group, the activity of SOD in the hippocampus of of I/R group was significantly decreased (P < 0.01), and the content of MDA in the hippocampus of I/R group was significantly increased (P < 0.01). Compared with I/R group, the activity of SOD in the hippocampus of I/R+ PROG group was significantly increased (P < 0.05), and the content of MDA in the hippocampus of I/R + PROG group was significantly decreased (P < 0.01).</p><p><b>CONCLUSION</b>PROG could improve the learning and memory ability following global cerebral ischemia/reperfusion injury in rats, the protective mechanism might be associated with down-regulating the expression of P2X7 receptor protein and attenuating oxygen-derived free radicals in the hippocampus.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of taurine on the ultrastructure and P2X7 receptor protein expression in brain following traumatic brain injury (TBI) in rats.</p><p><b>METHODS</b>Forty male SD rats, were divided randomly into four groups that were sham-operated group, TBI group, TBI plus low-dose taurine group and TBI plus high-dose taurine group. The TBI model was established by Marmarou's method, the expression of P2X7 receptor protein in parietal cortex and hippocampus was detected by the immunohistochemical method, the ultrastructure of parietal cortex were observed by transmission electron microscope.</p><p><b>RESULTS</b>Compared with sham-operated group, the positive expression cells of P2X7 receptor protein in parietal cortex and hippocampus of TBI group were significantly increased (P < 0.01). Compared with TBI group, the positive expression cells of P2X7 receptor protein in parietal cortex and hippocampus of TBI plus low-dose taurine group and TBI plus high-dose taurine group were significantly decreased (P <0.01 or P <0.05). Compared with TBI plus low-dose taurine group, the positive expression cells of P2X7 receptor protein in parietal cortex and hippocampus of TBI plus high-dose taurine group were significantly decreased (P < 0.05 or P < 0.01). The pathological damage of parietal cortex in the TBI plus high-dose taurine group was obviously lightened.</p><p><b>CONCLUSION</b>Taurine exerts the neuroprotective effect on TBI in rats, the protective mechanism might be associated with down-regulating the expression of P2X7 receptor protein in parietal cortex and hippocampus.</p>

ABSTRACT

Objective To investigate the dynamic change and associated risk factors of HIV sero-conversion rate in Beijing.Methods 809 sero-negative men who have sex with men (MSM) were recruited in the cohort from August to December in 2009.HIV sero-antibody,medicinal examination and behavior questionnaire interview were carried out every six months.Results 962 MSM with overall baseline prevalence of HIV infection as 6.34% (61/962) together with 809 seronegative MSM,were enrolled in the cohort.Of the 809 sero-negative participants,95.1% (769/809),85.5% ( 692/809 ) and 71.0% ( 574/809 ) of them were retained in the 6-month,12-month and 18- month follow-up visits,with 19,29 and 17 of them became HIV sero-conversion at 6-month,12-month,and 18-month follow-up visits and the HIV incidence rates appeared to be 5.47,12.37 and 6.86 per 100 person-years respectively.The HIV incidence was 7.59 per 100 person-years in the 18 months follow-up visit.Factors including:younger than 25-years old (HR =2.32,95%CI:1.39-3.87),having more than 8 MSM partners (HR=2.50,95%CI:1.49-4.20),less than RMB 2000 Yuan every month income (HR=1.76,95%CI:1.06-2.95 ),having more than 4 homosexual partners in the last six months (HR=3.50,95% CI:2.11 -5.81 ),showing phimosis and redundant prepuce (HR=2.47,95% CI:1.50-4.07 ) as well as positive syphilis test (HR=2.62,95%CI:1.53-4.49) etc.,were significantly associated with HIV incidence.Conclusion High HIV incidence was shown among MSM in Beijing and had spread fast in this population,calling for more favorable prevention measures to be taken.

ABSTRACT

<p><b>OBJECTIVE</b>To study the incidence and risk factors of HIV and syphilis seroconversion among men who have sex with men (MSM) in Beijing.</p><p><b>METHODS</b>A total of 550 MSM were recruited on the basis of community and followed up after 6 and 12 months in Beijing. Each subject was investigated by only one investigator at one time to collect information on demographics and behaviors. Blood samples were collected to test HIV and syphilis seroconversion. ELISA was used for screening test, west blotting (WB) and Particle agglutination were used for confirmatory test.</p><p><b>RESULTS</b>A total of 550 MSM investigated, among which 4.5% (25/550) were HIV-positive and 29.3% (161/550) were syphilis-positive. For 525 HIV-negative MSM, 87.0% (457/525) retained during the 12-month investigation. Seroincidence for HIV and syphilis were 3.37/100 person-years (95%CI = 1.66 - 5.08) and 9.32/100 person-years (95%CI = 5.87 - 12.77) respectively. HIV seroconversions for those who performed and did not perform rectal douching after homosexual anal intercourse in the past 3 months were 7.11/100 and 0.76/100 person-years respectively. Multivariate Cox regression analysis revealed that rectal douching after homosexual anal intercourse in the past 3 months (HR = 9.23, 95%CI = 2.08 - 40.88) was significantly associated with HIV seroconversion. Syphilis seroconversions for those who met male sex partners in parks, public washrooms or bathhouses in the past 3 months were 41.77/100 and 7.97/100 person-years respectively. Syphilis seroconversions for those who performed and did not perform rectal douching after homosexual anal intercourse in the past 3 months were 16.17/100 and 4.92/100 person-years respectively. In the past 3 months, meeting male sex partners in parks, public washrooms or bathhouses (HR = 4.67, 95%CI = 1.77 - 12.34) and performing rectal douching after homosexual anal intercourse (HR = 3.09, 95%CI = 1.40 - 6.83) were significantly associated with syphilis seroconversion.</p><p><b>CONCLUSION</b>The seroconversions of HIV and syphilis during the follow-up visits in this MSM cohort study in Beijing were very serious, and that the associated factors for seroconversions were rectal douching after homosexual anal intercourse and meeting male sex partners in parks, public washrooms or bathhouses.</p>

ABSTRACT

Objective To study the prevalence and risk factors of human immunodeficiency virus (HIV) among men who have sex with men (MSM) in Beijing. Methods A sample of 550 MSM was recruited between March and June in 2008 in Beijing. Interviewer-administered interviews were conducted to collect information on demographics and behaviors. Blood samples were collected to test for HIV,syphilis,HBsAg and HCV infections. Chi-square test and logistic regression model were used to identify risk factors associated with HIV infection. Results Of the 550 eligible MSM surveyed,HIV prevalence appeared to be 4.5% (25/550). Data from multivariate logistic regression analysis demonstrated that factors as reporting unprotected anal intercourse with causal male sexual partners in the last month (X~2=11.381,P=0.001),rectal douching after homosexual anal intercourse in the past three months (X~2=5.326,P=0.021),feeling sad in the last month (X~2=8.809,P=0.003) were significantly associated with HIV infection. Conclusion Health education on safer sex behavior and mental health care should be taken to hinder the speed of HIV transmission among MSM.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of progesterone on matrix metalloproteinase-3 (MMP-3) expression in neonatal rat brain after hypoxic-ischemia.</p><p><b>METHODS</b>Followed the hypoxic-ischemia of neonatal rat brain, Evans blue (EB) staining and transmission electron microscopy were used to detect the blood-brain barrier pathological changes on permeability. MMP-3 protein expression in cerebral cortex was measured with Western blot.</p><p><b>RESULTS</b>Transmission electron microscopy results showed that the blood brain barrier in hypoxic-ischemic group changed significantly compare to progesterone group. EB staining results suggested that the blood-brain barrier permeability of hypoxic-ischemic group was significantly increased compared to sham-operated group (P < 0.01). The blood-brain barrier permeability in progesterone group was also decreased in comparison to that of hypoxic-ischemic group (P < 0.05). Western blot image analysis results indicated that MMP-3 protein expression in the hypoxic-ischemic group increased significantly than that in sham-operated group (P < 0.01), and the progesterone group was decreased significantly than that in hypoxic-ischemic group (P < 0.05).</p><p><b>CONCLUSION</b>Progesterone may reduce the blood-brain barrier damage by reducing MMP-3 expression. This might be one of the protective mechanisms in the hypoxic-ischemic brain injury.</p>

ABSTRACT

The aim of this study was to investigate the effects of 2-methoxyestradiol (2-ME) on expressions of caspase-3 and survivin in chronic myelocytic leukemia (CML) K562 cells. The experiment was divided into 3 groups: control group, in which K562 cells were cultured in medium without 2-ME; the experimental group, in which K562 cells were cultured in medium containing different concentrations of 2-ME (1, 2, 4, 8 and 16 micromol/L) for 36 hours; the negative control group, in which K562 cells were replaced by distilled water without RNase in medium. The apoptosis rate, the protein and its mRNA expressions of caspase-3 and survivin of K562 cells was detected by TUNEL, flow cytometry (FCM), half-quantitative RT-PCR respectively. The results showed that the apoptosis rate of K562 cells in experimental group was significantly higher than that in control group (p < 0.05). The apoptosis rate of K562 cells detected by FCM was almost the same as that detected by TUNEL method (p < 0.01). The result detected by TUNEL methods was positively correlated with that detected by FCM (gamma = 0.845, p = 0.034). The expression of caspase-3 protein increased in a concentration-dependent manner, and also this expression level in the experimental group was higher than that in the control group (p < 0.05); the expression of survivin protein decreased along with the increasing of 2-ME concentration. and the difference between the experimental group and the control group was statistically significant (p < 0.05). The expression of caspase-3 mRNA was higher in the experimental group than that in the control group (p < 0.01), and the expression of survivin mRNA was lower in the experimental group than that in the control group (p < 0.01). The expression level of caspase-3 mRNA was negatively correlated with that of survivin (gamma = -0.966, p = 0.001). It is concluded that the 2-ME can induce apoptosis of K562 cells in a concentration-dependent manner and indicate its promising potential in the treatment of CML patients.
